Application Form
The information sort from the application form is in accordance with that required for reporting purposes to the National Regulator. There is also information for the purposes of Hamel Institute to support the provision of quality goods and services to the client. The client may print this form and mail it to Hamel Institute to progress their application.
The client should not provide more information than is required.
The date of birth, legal name and unique student identifier (USI) of a client is required to award a qualification or Statement of Attainment.
All clients need to accept and acknowledge acceptance of the Privacy Notice and Student Declaration Form to complete the process.
